Complete freeze and no response 0x00000124 ntoskrnl.exe+4b297c

This started happening in late november/early december when i replaced my old GPU - gtx 650ti - to a new one gtx 960.

    current pc:

    CPU Intel Core i5 4690 4x3.50 GHz
    MBO Gigabyte GA-B85M-D3H
GPU Palit GeForce GTX 960 Super Jetstream 2GB
RAM Kingston HYPERX Black 2x4GB 1600MHz
    PSU Be Quiet! PurePower L7-530W
    DISPLAY 16:10 iiyama E2207WS & 4:3 LG L1732P

this is a non-oc build. the card is factory overclocked but i didnt do anything to it too

everything else was the same hardware wise (that build is 1 year old). reinstalled windows and installed all the programs i would normally use.

on the next day my pc froze up completely with a looped noise during a league of legends match. unresponsive, didnt restart, nothing. i had to manually power it down and still it took a while to respond to a reset. when i rebooted the pc the choose operating system choice came up. i started windows normally and i was surprised that there was no message like "windows recovered form a serious error etc...". there was no such thing. i thought that something got screwed up with drivers, reinstalled them. it didnt happen again untill next week when i was playing starcraft 2. now that started to look suspicious. i was worried that the new GPU is causing problems. before i buy any components i use a few online power calculators to see if my PSU will handle it. it was fine. way above the power i needed for that system.

so a week later it starts to happen more often. it always occurs doing playing a game. most of the time its when i play path of exile (a better version of diablo, both cpu and gpu intensive). to the point that after new years it happened 6x in one day. sometimes my screens shut down for a second and then turn back up. after a few minutes the pc freezes ocmpletely again and is unresponsive -> restart manually only. now i knew something was definetely borked so i looked into hardware problems. read some stuff online what to do. how to create minidumps and how to read them. but here is the funny part, the only time a minidump was created was on Jan 3rd and i didnt have 256 mini dumps enabled then, so the only minidump available is the default one.

    what i did since then:

demounted CPU heatsink. i had a OEM kit for the processor, so the cooler was from intel and the paste that they had on the cooler completely dried out to dust. i thought that was the cause of the freezes. dusted out the whole pc, put new thermal paste on cpu (arctic mx-2) -> temperatures dropped on the CPU, but still crashing -.-

ran memtest86 all tests, different configurations -> no errors

deinstalled nvidia drivers clean with DDU, downloaded newest ones and installed -> still crashing

verified game file integrity on all gmes that crashed -> 100% checked, still crashing

did sfc /scannow -> no errors

declocked my gpu a bit to see if its causing the crash (its factory overclocked, all my pc is stock, im not overclocking anything ever) -> didnt work, still crashed, reverted to defaults.

temperatures of cpu go up to 60-65 C at max load
gpu temp does not go above 50

the only game that does not crash my pc is CSGO.
    fullhd/bluray movies run fine

all games run perfectly smooth untill they crash. no performance drops beforehand.

im really feeling depressed because of this. before i exchanged the gpu everything worked fine, then i switched and got kicked in the balls.

as i mentioned previously the only minidump in the windows/minidump folder was the one from jan 3rd and its not a 256-one, its a default one. no minidumps are created when it freezes. i uploaded the dm log but i assume not a lot will be made from it

i will also include what bluescreenview said about this minidump

uninstalled daemon tools lite and the pc froze during playing. before the freeze there was a clear performance issue for 5. this time the screen on one monitor went yellow-brown colo and the main monitor went completely black.

could this be an issue with the GPU?

You are running an ASUS Product ID (PID) on a Gigabyte board.
    This makes your installation counterfeit.
Troubleshooting a counterfeit installation is useless as it is not known what has been modified to your system.
A counterfeit installation contains heavily modified code which causes the system to behave in unexpected ways.
For this reason, analyzing counterfeit systems is unreliable and most analysts won't try it.
    Code:
    OS Configuration: Standalone Workstation
    OS Build Type: Multiprocessor Free
    Registered Owner: Canz
    Registered Organization:
    Product ID: 00426-OEM-8992662-00173
    Original Install Date: 2015-11-26, 16:04:57
    System Boot Time: 2016-01-07, 21:04:40
System Manufacturer: Gigabyte Technology Co., Ltd.
I recommend to install a genuine copy of Windows.
If, after you have installed a genuine copy of Windows, you still get BSOD's we'll be happy to assist you.

ok replaced the gpu with the old one. played for 6 hours now no freeze, no glitch. but i noticed something funny. cpu load and temperatures while playing go to 100% sometimes and 60-66 degrees.
also while in game i launched skype to start, and it took so much power on my pc to just start it up. the main screen in which i had pat of exile (game) loaded up blinked for a second and untill the skype was fully loaded it ran very slowly, high cpu load basically.
also no idea if its supposed to be like that, but when i turn my pc on from complete shutdown, the gpu fans start immediately when i start my PC, but the CPU fan launches only after a second or two, spins up to 100% and slowly torques down to stable rpm (i assume its like 1000rpm)
