It mainly happens in areas around alot of particle effects such as the Demon Ruins or the Crystal Caves.

They look to be caused by your Xbox drivers, the installed version is known to cause problems.
Check the manufacturer for driver updates.

Are the driver links you put in there the drivers i need to go install?
And just to clarify you want my to install my xbox 360 controller drivers and not games for windows live or anything.
EDIT: My xbox 360 controller drivers are up to date.
EDIT2: Assuming the 21 in xusb21 stands for version 2.1, is it better to downgrade the version of my xbox360 drivers then use the 2.1 drivers?
Just to note, you were probably seeing adds.
Either update the drivers or keep the device disconnected.
There are no updates for the 360 controller drivers, and dark souls w/ keyboard and mouse is the worst thing ever.
I know Dark souls is impossible without a controller, but the drivers of the controller are the cause. I do not have options for you that resolve the problem without the removal of the device or something.
You could remove the device from device manager and reboot so the drivers will be installed again on reboot, but I honestly doubt that it'll help.
