    BSOD Stop c000021a {Fatal Error} verification of known dll failed


    I have see many posts on this but i am still stalled and wanted to see if i could generate an aha moment from someone.
    I am sorry for the length but I have tried many things before posting this.
    Here is what I have done so far:

    Start 1. F8 disabled auto restart on system failure.
    Startup failed –BSOD
    Stop c000021a {Fatal system Error}
    The verification of a known dll failed system process terminated unexpectedly with
    A status of 0xc00012f (0x00322780 0x00000000)
    The system has been shut down

    I shut down the computer

    F8 at startup

    Restart 2 attempted to use last known good configuration
    Startup failed –BSOD
    I shut down the computer

    Restart 3 attempted boot into safe mode
    Startup failed- BSOD
    I shut down the computer

    Restart 4 dissable drivers
    Startup failed – BSOD
    I shut down the computer

    Restart 5 - Bootlog (creates ntbtlog.txt)
    Windows appeared to be starting – Logo appeared and remained on screen for
    Over 20 mins then BSOD

    Restart 6 - Directory services restore mode
    Startup failed – BSOD
    I shut down the computer

    Restart 7 - debugging mode
    Startup failed -- BSOD
    I shut down the computer

    Restart 8 – Low video resolution
    Startup failed -- BSOD
    I shut down the computer

    I then ran the win 7 disc WRE

    Start 1 Startup repair
    Could not repair automatically
    Problem Event Name startup repair offline

    Problem Signature 01 0.0.0.0
    Problem Signature 02 6.1.7600.16385
    Problem Signature 03 unknown
    Problem Signature 04 2199870
    Problem Signature 05 auto failover
    Problem Signature 06 39 (I have tried this a number of times hence the 39)
    Problem Signature 07 Corrupt File

    OS version 6.1.7600.2.0.256.1
    Locale ID 1033


    Start 2 System Restore (Only 2 restore points were offered)
    "System restore attempted to restore the system for several minutes."
    System restore did not complete successfully. Your computers files and settings were not
    Changed.

    The details section displayed.
    Details:
    System Restore failed to extract the file (D:\Users\Dan\Downloads\Bytecc_floppy_
    Disk_Drive_Driver_update_01-2014.exe) from the restore point.
    The restore point was damaged or was deleted during the restore.

    (The bytecc floppy was an external floppy drive i attempted to incorporate into the system for archiving some old stuff, but it never really worked. but that was a while ago )

    I tried both of the restore points that the system offered but they both failed.

    Start 3 Command Prompt
    Attempted SFC /scannow
    Failed There is a system repair pending which requires a reboot to complete. Restart windows and run sfc again.
    I tried this a number of times and received the same message.

    Start 4 Command Prompt
    Ran chkdsk /f
    Error cannot run because the volume is in use by another process. It may run if this volume is dimounted first. Force dismount y/n
    After I said no a couple of times then I tried yes

    Ran chkdsk /f
    Windows replaced bad clusters in file 259751 of name\windows\prefetch\readyB~1\tracc6.fx

    Start 5 ran sfc again
    Failed There is a system repair pending which requires a reboot to complete. Restart windows

    Start 6 tried system restore again
    Same error as before.

    Start 7 Ran startup repair again
    Did this a number of times with the “Could not repair automatically” error

    Start ??? Ran startup repair again
    This time there was a different msg.
    Could not detect a problem

    Diagnostic repair log
    Last successful boot time 5/15/2016 10:18:47 am
    Number of repair attempts 43 (said I tried this a lot)

    The report is lengthy but the was one issue

    In the details report many of the test came back with a, “OS booted successfully” msg

    So that is it so far. Anyone think I have a prayer of resolving this. I also cloned the drive just in case. My files seem to be in tact as I am able to see them when I attach the clone to my laptop externally.. but I would rather not rebuild the system from scratch..

    Hi DC,

    This looks like your hard drive is causing the problem, safe mode is a strong indication of hardware & chkdsk finding bad sectors also is a good indication towards the hard drive.

    If possible I would ask you to run SeaTools to check the integrity of your HDD. SeaTools for DOS and Windows - How to Use - Windows 7 Help Forums

    I ran the Seatools diagnostic, (dos version) and both the short and long test came back with no errors... acoustic seemed negative as well. (Fans were louder)

    Could you run following command please
    Code:
    chkdsk C: /r
    C: is the Windows partition letter, if yours is different use the one you have configured.

    /r includes /f, /f marks bad sectors as bad where /r also attempts to recover readable data.

    Hi just ran chkdsk /r
    Result: Windows has checked the file system and found no problems.
    also: There are 0KB in bad sectors

    An important thing not to forget, what happened before the BSOD occurs?

    Could you please provide a screenshot or photo of the disk management in command prompt.
    To do this enter following commands
    Code:

    list disk
    sel disk 0
    list partition

    Hi.
    After system POST then the
    starting Windows splash screen appears and appears to be loading the operating system.. (this lasts for about 45 secs or so) then BSOD flash and it starts the process all over again.
