BSOD atikmdag.sys while playing games

Heya, computer has recently started giving me troubles with the blue screens of atikmdag.sys with my graphics drivers failing and recovering between.

I've uninstalled/reinstalled all my drivers multiple times and sometimes it fixes it for a while, but it always ends up coming back. I've redirected and changed the sys files to avoid the BSOD from some online guides and still nothing, just keeps crashing when I play any game.

Not sure if it's the graphics card/drivers dying on me since my rig is 5 years old and my graphics card is about a few generations behind, or my MOBO crapping itself. I included the ZIP file as requested, hope we can find this out!
