Would format c-e be the culprit?

You put the image on the drive and it boots OK (says to me that the image is OK)
Then, when you go to reboot, it doesn't work.

I'd suspect that that would be caused by a hardware error.
It's possible (but highly unlikely IMO) that a format could help out - but I'd go with KillDisk/DBAN first, then the format.

Run a manufacturer's diagnostic on the hard drive to check for physical problems. There's a list of them here: HD Diagnostic

I suspect the hard drive. Try this free hard drive diagnostic: HD Diagnostic

The image seems OK, but certain operations don't - so that says to me that it's something physical with the hard drive.

If you suspect malware, be sure to run a scan with Malwarebytes.
