Need help with logo screen

Hello, as the title says i need some help with this login or logon screen i found over on another forum heres the link--------- http://yournumbr1fan.deviantart.com/...F25005588&qo=0 hopefully you guys can help on how to set it up i read in the instructions that you need to use a logo editor........
