This are system requirements for GTA4,so I think that you can play it on High.

    GTA 4 Minimum System Requirements

    • Memory/RAM: 1GB for XP and 1.5GB for Vista
    • Video Card: 256MB NVIDIA 7900 or 256MB ATI X1900
    • Hard Disk Space: 16GB free
    • Operating System: Windows XP SP3 or Windows Vista SP1

    GTA 4 Recommended System Requirements

    • Processor: Intel Core 2 Quad 2.4GHz or equivalent AMD Phenom X3 2.1GHz
    • Memory/RAM: 2.5 GB for XP or Vista
    • Video Card: 512MB NVIDIA 8600 or 512MB ATI 3870

According to what I have read about GTA4, the game is a poorly optimized console port to pc so, even the High End Gaming PC had a hard time playing it in High Settings... buggy coding = resources hogger.

Why am I saying this? Just so you won't blame your pc

You'll be able to play it but don't expect to max it out with the 5650. Start with low settings and work your way up to find the sweet spot for your system. Also if the game performs horribly with the latest patches try to revert it to patch 1.0.4.0 if possible and try that. I don't advise even trying the ENB until you get the game running good for your system first.
