I´ve found something, it could be that this same thing is a few posts back, but i am to lazy to look for it.
This is what i´ve found on Google! :P.

For the PC version of the game, the problems seem to be bigger as the game crashes when it starts, and even though there are no official answers for the problem at the moment, it appears that the problems are caused by the soundcards (the 3D surround options in Fallout 3, to be more specific).

So, if you have Asus Xsonar, Creative or Realtek sound cards and your Fallout 3 game crashes when you start a new game, you can try the following trick reported by Bethesda forum user zsmono:

Just turn off the GX mode if you have a Xonar card. “For the others. This DS3D GX thing is part of Xonar’s drivers which is responsible for restoring EAX and multichannel sorround sound under Vista. Similar to Creative’s Alchemy or Realtek’s 3D SoundBack. Try to disable them, or change the speaker config to stereo.”

    Possible solution

hey, honestly i had the same problem with the game...Over like 10 re-installs this is the only thing that has worked for me so far with no crashing....Install the game fresh install right...after u install do not play the game yet...install the update from Bethesda website...I did not make an icon on the desktop instead i run the fallout luncher directly from the program files folder...I also run the game in windowed mode....not the best gaming experience but in windowed mode ive had way fewer crashes. hope it works for you bud

This may help somebody else... who knows. I trudged through all 40 pages of this as well and nothing worked and I ended up doing a fix on my own...

I discovered my FALLOUT.ini had maybe 2-3 lines in it for whatever reason... And I reverted back to the ol UT days where editing ini's were "the thing"

So I deleted the .ini knowing the game recreates it and waalaaa it woorrkedd.

My initial issue was the CTD on load. I had done most of the troubleshooting mentioned here already but filled in the gaps with all the other little solutions and nothing worked.

Again.. somehow my fallout.ini had a few lines in it and wasn't completed... Once I deleted it and let the game recreate it for me, the game loaded perfectly with no issues and ive played a few hours with no glitches.

who knows. It happened to me so could happen to someone else?

Here is a fix for windows 7 and fallout 3 CTD

The issue is not with video drivers, its with .netframwork 3.5.1. And to fix this you need to do this.

    1. Go to control panel, programs and Features, then Turn windows features on or off.

    2. Then click on the + next to Microsoft .NET Framework 3.5.1 and put and check mark on the box below that!!

    3. Click on ok then if it ask you to reboot then reboot and then try the game again and it should work!!
