open application icon is missing
I've met with that problem since beta ver...
And it's still take a place in RTM
is there a fix for it ? (attached screenShot explain what i'm talking about)

Same here, it's been like that since beta, and still is. Haven't seen any quick fix for it.
Seems to be only with executables, there is no specific icon being associated with them.

Yep, it's from as Aaron posted that it's a bug and there is only the default unknown file icon available for this "Open" toolbar icon with exe or executable files since they usually have their own individual default icons instead.
I think the desired behaviour would be to actually show the icon of the .exe that is being hovered/selected which I think may not be possible since the only association with an .exe file of any kind would be explorer. I could be wrong though. Here's a solution.

Thanks for the solution that was annoying me for a while, and it's works if there would be an open exe icon in dll library this problem wouldn't happend usual microsoft problems, we can choose another icon but I don't think windows would use the selected exe file's icon next to open command it had to be programmed that way which would be much better
