Did you already try going through the process in the tutorial below to manually change the shortcuts' icons to it's default icon a couple of times to see if it may reset them to be correct afterwards?

I think this is BUG because EVERYWHERE in all dialogs: properties, change shortcut, delete shortcut or file, run as administrator, in the application window i can see an icon of these several applications: CPU-Z, TCPView, FileRecovery and some others, BUT in Explorer and on the Desktop there is no icon of the applications.
Why if this is not a BUG Windows can display an icon in other dialog windows? If it would be the damaged icon of the application then Windows could not display an icon in any other places...
This happens on my PC (randomly).
Programs that haven't been moved or deleted sometimes lose their icons.
If I check the "Properties" window, I can see the correct icon.
Reapplying the icon doesn't fix the problem.
On my PC, restarting fixes the problem.
It's not a defect... if it was, many more people would be complaining at this late stage. You may have some peculiar version problem with system files. Do you recall about when it started happening? You might want to roll back to a restore point prior to this. If that's not possible, consider applying the latest service pack again and then accepting the updates that follow.
I suspect that certain combinations of hardware, software and Shell Extensions are to blame.
IMO, it's just like how some people state that certain programs always crash and yet other people never experience any problems.
It may happen more often than it is reported because the user has to notice the icon is missing.
On my PC, I generally only see this in the Start Menu and it is usually when I am searching for a program that I don't use on a regular basis, or that I normally start some other way (like from the Taskbar).
For example, occasionally I have noticed that the Firefox icon in the Start Menu is broken, but normally I run Firefox via a batch file that is pinned to my Taskbar.
