Sleep mode means your user session is kept stored in the memory (RAM) while everything else (other than LED lights) is turned off. If you unplug your PC while in sleep mode, you lose all unsaved work since RAM is not able to store data without electricity unlike Hard drives.
Hibernate means the user session that is stored in the memory is stored in a file called hiberfil.sys and then Windows completely turns off all the devices and the computer itself. You can uplug your PC safely by using this mode. When you turn on your PC, you basically get to your previous user session.
Shutdown is self-explanatory.

With my desktops I prefer S3 Sleep as it's very fast waking up and the Windows session is in the same state as it was before going to sleep.
I like to be able to walk away and not worry if I'm coming back. So I set it to sleep at 30 minutes, then Hibernate at an hour. I also enable Hybrid Sleep in Power Options>Advanced so if power fails during sleep it has already been written to the HD so nothing is lost.
I especially like having the pC start twice as fast from Hibernate and have my work out as I left it. I even use this in place of Shutdown at the end of the day, after checking if any Updates are cued on the Shutdown button on Start menu.
