    Start with this

SFC /SCANNOW Command - System File Checker.

    Please fill in your specs

System Info - See Your System Specs

Make sure you have up to date video drivers, and I would recommend scanning for malware

In addition to what Dude has recommended, I have had luck doing a diskcheck (CHKDSK /F from elevated command prompt). I've seen this find and fix errors and alleviate the explorer.exe is not working problem.

No, read Option Three of this tutorial which you followed earlier - SFC /SCANNOW Command - System File Checker

The dump file is a few months old so the findings are most likely going to be invalid now, but I'll have a look just as a matter of interest.

    Code:

    Probably caused by : CHDRT32.sys ( CHDRT32+11f82a )

It seems that your Conexant High Definition SmartAudio driver is causing problems, by not using pointers properly, leading to access violation exceptions which do not have the appropriate exception handler. I would suggest checking for any updates from here - Conexant - Support
