3 Instances of My Documents, with their Names not showing correctly

I copied and changed C:\users\xxx\My Documents to the 'D' drive, in this format D:_My Documents, as shown in the first image.
(1) The 1st instance is the copy at D:_My Documents: note there is no underscore in front of My Documents in the Nav pane.
(2) The 2nd instance is the original C:\users\Dan\My Documents.
(3) The 3rd instance gets an Access is Denied error, but is at same location as (2). What is this folder?

The following refers to the second image.
On D drive, My Documents displays as My Documents (no leading underscore), yet the underscore shows in the url when My Documents is selected.
When I change the display image of D:\My Documents to D:\_My Documents (which it already is), it is then changed in the display and on the Nav pane. WHY?
Where can I find out how Win7 decides how to classify names in libraries, since My Doc's is one of the main libraries. Is this even a Library issue?

Windows 7 Ultimate x64 SP1, Windows 8.1 Pro x64, Windows 10 Pro x64

The last My Documents is a Junction and it is there to allow legacy programs to interface with Windows 7 folder structure.

You can delete the second (original My documents) after you reboot. I have found that when you move the location of My Documents, it leaves the original empty folder sometimes. Before you delete, check to see that no files remain in the original.

I cannot help with the second part of your query, sorry.
I have never seen that leading underline notation associated with My Documents.
I normally change the name to just Documents.

Hello gypsycomp,

In addition to what pbcopter posted above, you could use OPTION TWO in the tutorial below to restore the default location of your My Documents folder.

User Folders - Restore Default Location

It would be best to not have hidden folders shown to avoid messing with the My Documents symbolic link with the shortcut arrow on it.
