How many pins on the s-video connector on that card? If it is the normal 4 pin (or even 7 pin) then one of these,

If that is the case all you need is a plain old s-video cable long enough to go between the two.
This on XP or Windows 7? If it is Windows 7 there is a good possibility it will never work seeing as there is not now and never will be a Windows 7 driver for that card; it is just too old and the last driver update it saw was for XP from back in 2006,
