How to remove a Bluetooth device?

The Bluetooth device list always shows my older phone which is already broken long time ago. When I delete it, it re-appears after a while, or possibly after a reboot. How can I permanently remove it?

Have you uninstalled it in device manager? You may have to show hidden items under the view section.

Stumped - Add Device or Remove Device Not Working
HI I have searched high and low for a resolution. OK. When I went to add a new Bluetooth keyboard (Control Panel>Add Devices). When pressing Add Device, nothing happens. I have two Bluetooth devices already installed, are listed under "Devices" are "found" and work fine.

Bluetooth Device won't appear in Device Manager
I installed The Bluetooth/Wifi module that came with my mobo (DP67BG), after setting the drivers i can only use the wireless adapter while my bluetooth device won't appear. Any help will be appreciated, just note that i redownloaded the BT drivers but nothing changed.
