constant chkdsk with no error codes

Been up for 24 hrs trying to fix this machine and honestly i'm thinking it's more of a hardware issue than operating system problem. I was asked to work on a machine that keeps deleting the operating system files on every restart. I did a reformat and it still does it. there are no error codes just starts up and starts a chkdsk which starts deleting files.

The hardware that I know that's on it is:
    CPU- Intel i3
    MB- Intel DQ67EP
    HDD- Seagate st1000dm003-1er162
    Opt Drive- Tst corp ts-h553a
    Graphics card- Xfx geforce 7600gt

I have tried installing all drivers but the MB driver tells me that the hardware can't be found when installing. I'm not hooked up to the net yet ( trying to avoid any other complications w/ updates atm). If someone could help me with this I would appreciate it. Thank you in advance

First job would be to get a disk by going to disk makers website and getting there test software windows test generally for file system errors the makers test the actual drive. Once you install create and disk image you can then get it back in a few mins save reinstall.
It doesnt normally dlete files there should be check disk found files. Did you delete all paritions and leave the disk blank and let windows create new ones there could be a fault at the start of the drive that the makers test can pick up
