Hard Drive disconnects from PC at random

So I've been having issues with it for a while and that includes
Hard disk running slow and not responding
Hard disk disconnecting from the PC whenever it decides
sometimes prompts me to format it
I checked my system files in the windows logs and I get this error message every time the disk disconnects

    error atapi 11
The driver detected a controller error on \Device\Ide\IdePort0.

The disk might be dying so just in case I tried to transfer files from it to another hard drive but it doesn't let me do that because it stops responding and again disappears and I have to reset the pc for it to appear again
I tried replacing cables for the disk but with no help at all

    Any suggestions on whats happening?

    I am using Windows 7 Ultimate 64-bit

The controller is bad. The drive may not be bad.

Try a different drive cable. If that doesn't solve it, then swap this drive with a known good drive from another computer. See if the problem stays with the computer, or if it goes with the drive to the new computer.
