Missing appointment for both admin and delegate

Hi There,

So we are having an issue here, where there is a missing appointment.
The user have a delegate who accept this meeting on his behalf.
I have a feeling its a user error but I just want to reconfirm everything.
They are using MS Office 2013.
When an appointment is sent the other attendees can see the appointment in their calendar except this user.
We thought its a local issue with his device, but his delegate can not see this appointment as well on their end despite double checking that this has been sent to this user as well(ensured correct email is use).
The issue is intermittent so its quite difficult to replicate the issue.

Appreciate all your help.
