That can be tricky to figure out. Reset BIOS back to default and start making changes one by one and checking. Sometimes one setting will be something prime doesn't like. Or as a quick test bump up the vcore. I'm guessing you are running blend. Try small fft's and see if it will run.
I've run CPUs for years that couldn't pass Prime, and never had a BSOD. I know that is blastphemy in an overclocking forum, but it's true. I have also passed 12 hours of Prime, 12 hours of AIDA and 20 passes of IBT on max. Put a DVD in and started Handbrake and had a BSOD within 30 seconds. So, I still use the artificial tests, but I've concluded they don't necessarially prove you are stable. So far, I've never found an application that is better for stressing a CPU than Habdbrake. And it's a real useful program and not an artificial test. That's my story and I'm sticking to it. Remember how many had problems running the ROG Real Bench? That's because it used Handbrake. I use Handbrake frequently.
Yup I have the BCLK at 101 CPU offset +.060V. I'm trying a "-" of .020-.040 now with BCLK at 100, multiplier back to 45.
Gary, set your CPU to manual not offset and set a 1.25 voltage with the bclk at 100 multiplier at 45 and try that. Then start moving the voltage up or down depending on your results until you find the lowest voltage you are stable at. What is your VID? Once you find the lowest voltage under prime and know the VID under Prime, the offset is fairly simple to figure out. What do you have LLC set to?
Gary set LLC to Ultra High. See if that makes a difference. Also, read this and set your BIOS exactly like it is except the ram settings and voltage, but leave the CPU Voltage to manual and try again. Ivy Bridge Overclocking Guide ?
