I personally use and install Clean My PC 4.37 for that purpose. It has a registry cleaner AND a defragmenter to 'fix it up'.:)

Also, I stand by it since I have it installed on about 12 separate machines and it always resolves Windows performance issues. The fact is that the Windows registry does get messed, but that doesn't mean that re-installing Windows is the only option. I would die.
