I replaced the sata cable and now I only attach my Transcend USB 3.0 card reader when I have to use it instead of leaving it connected all the time.

I performed the 8-pass memtest about a year ago when I got new ram and all tests were fine. Can ram go bad over time?

I'll do the disk check and physical surface test next.

Thanks very much for the help.

Yes mate it can go bad as you say through whatever reason I mean they are essentially only blocks of chips with millions of transistors on and are exposed to a lot of use over time and heat of course and being so very small damaged easily.

You could try reseating them though and this I use.

    SLOT CLEANING

Now as for the slots to clean themuse a strip of old credit card about 1/2 inch wide nip the corners off one end- preferably round them offhold a piece of thin lintfree cloth over the end dab on some form of alcohol - isopropyl alcohol (whatwe swab the skin with before an injection) is good and GENTLY swipe along the slots.

This can also be done for the GPUslot/s and also the contacts of the sticks or cards - not the stick but somelint free cloth of course. See my pic for the sick but please be very gentledoing it if you are in any doubt - please ask before doing it but I use thisall the time and have yet to damage anything.

Sorry mate am not much good with the dump files but can you go through the Device Manager again and look at the keyboard and mouse again for those triangles.

Plus can you run the chkdsk I posted in post#2 and use the options / f and / r in Option 2.

This is interesting, you have an ACER/GATEWAY/PACKARD OEM on a Gigabyte board,

System Boot Time: 5/18/2014, 1:00:30 PM
System Manufacturer: Gigabyte Technology Co., Ltd.
    System Model: Z68X-UD3H-B3
    System Type: x64-based PC
