Overall slow performance, especially in games.

Hello guys. In the past months, I've changed up quite a lot of things in my PC. I've changed my GPU, cooling fan, MoBo, case, and everything is set up fine now (still going to upgrade my PSU though). But with the specs all fine, I'm having problems with PC performance.. Sometimes I experience very long waiting times while using my PC, for example sometimes Windows Explorer freezes up, or I am doing a few things at once and everything suddenly slows down..

I really don't know the cause, and could use some help here. The only thing that could be bad in my system is the PSU, which is quite old and most people say it's not recommended, as it's a cheap one.
I have switched mobos with sysrep /generalise /oobe, and everything went fine.

I have all the recommended specs for games, and it runs smooth, but when I enter crowded cities, I get dips to 30FPS. Now I have seen a video with a person on the same specs on e.g. Fallout 4, no OCs, and it runs smoothly at 60FPS. Something's not right in my system..
One thing I have noticed - my GPU and CPU usage is always really high (95-99%), even though most people say they have 70-80%.. My temps are fine, CPU idle is 39~ though, even with an aftermarket cooling fan, but only reaches 60~ while on said FPS drops, even. GPU is 50 idle and about 60-65 on load.

I have cleaned the dust on my PC (apart from the PSU).
MoBo temp is about 34. Using two case fans.

My drivers are up to date.

    My specs:
    ASRock Z75 Pro3
Inter-Tech Energon EPS 750W (the PSU, thinking of upgrading it)
    Intel i7-2600 @ 3.4GHz
    Nvidia MSI GTX 970 4G
    CoolerMaster Hyper 212 EVO
2 1TB HDDs (one SeaGate 5400RPM, one WD 7200RPM AFAIK)
    Zalman Z11 Pro Plus
    Windows 7 64-Bit

    Hi,
Well yea I'm sure a 5400rpm hdd isn't going to help matters much
You need to step up to at the very least 7200rpm's or even better a ssd is the bottom line :/
A lot of good inexpensive ssd's around MX or BX 100 Crucial's
Crucial MX200 2.5" 250GB SATA III MLC Internal Solid State Drive (SSD) CT250MX200SSD1 - Newegg.com

A 5400rpm hdd was in my low powered M-Series Gateway laptop
Since changed to a Crucial mx100 128gb ssd it's like night and day comparison :)

Sorry, my bad. It's a 7200RPM, I just checked up on my harddrive that has Windows installed.
Anyways, I have an HDD problem, but I doubt it's causing me the said performance issues.
I get a 0x0000000f4 BSoD, and have recently got a 0x00000007A BsoD (or something like that) that said KERNEL_STACK_INPAGE_ERROR. Now, I get the 0x000000f4 once a week.. I run chkdsk /f /r on C:\, and it fixes it for a week, and I don't know what to do... PC is performing OK but games are still slow...
It seems like games run smooth, but when it hits crowded cities for example, it becomes very slow, which is NOT supposed to happen.
