Yes, please edit your last post: copy that report to notepad, save file, attach using paper clip in reply box and delete the text from your post. That mess will drive helpers out of this thread.

You can try a Repair Install which reinstalls the OS while keeping all of your files, programs and settings in place. Then run sfc /scannow again to see if it replaced the damaged system files.

Now try running chkdsk. If it continues to fail, consider an OS reinstall, or use the System File Checker and Surface Scan on free Partition Wizard bootable CD.

Remember to also run the full HD diag/repair CD scan linked earlier to give your HD a clean bill of health.
