Cookies to keep? The version I'm using has the option to save cookies:

Toolbars - I mean the toolbar(s) that might get installed as additional software when you install Cccleaner if you don't uncheck the install options. They don't affect it's functionality or appear in the GUI. Previously it installed Ask Toolbar and Google Toolbar but I'm not sure what the current installer offers. I always download the Slim Build without toolbars.

The only extra options that I see that can cause problems is from running CCEnhancer as some of the available cleaning options can remove stuff that you don't want to remove. Here's a classic example:

With that option checked you lose all custom folder views! Anything with an asterisk is en extra entry that doesn't exist in the regular version of Ccleaner.

I've noticed that various installers offer you items based on the OS you are installing the software on.

I'd often be offered Chrome if I installed some software on XP, but the same installer wouldn't offer Chrome if I was installing it on W7.

No I always look for add ons I know what you mean now and I use Unchecky too which rarely misses that crap.
I only ever usethe default settings in CC and yes that is how I save cookies say for example this forum, banking etc etc I regularly use the reg scan too and save the results before fixing but in any case I always back up the reg after a clean install and before I tweak it as in Shawn's Optimise or any tutorial not that he or whoever is likely to give those tweaks if they were not safe but more if one hits a wrong button (which I also tell others to do when posting that tutorial) . Plus I have my regular images.

I've been using the portable version for a while now and haven't see any "add ons" when updating to a new version.
I prefer portable versions whenever possible, and i haven't had any problems with CCLeaner portable.
Updating one "portable folder" works for all OS's on my multi-boot test box, so i don't have to update 3 OS's one at a time for an installed version.

Actually David sounds like a good idea to have the regular free scanners on a portable device would save a it of disk space I suppose especially after filling my SSD full and wrecking the thing trying to claw empty space back
