That, my friend, is confusing. I have never seen a pc with your sound setting.
Try right clicking Nvidia HDA and select disable then see how your sound performs.

Most of the newer video cards have HDMI sound, make sure realtek is the default device

Do you know which windows update that installed and caused this? If so you can remove it

    Windows Update-Uninstall

    Edit

    You can try this too

    1 Go to control panel
    2. Double-Click on the Sound Icon
    3. Single-click the Speakers to highlight them
    4. Click on Properties (bottom right corner)
    5. Go to the Enhancements Tab and Click on it
    6. Now click "Disable All Sound Effects"

    saherboy,

Try this Click the Start Button, type "services.msc" (without quotation marks) in the Search Bar and press Enter to start Services.

In the right pane, please check whether the Windows Audio service is started.

If not, double click the service. In the tab "General ", make sure the "Startup Type" is "Automatic". Then click the button "Start" under "Service Status" to start this service.

If it is started, stop it, then restart it
