Yep I do have an optical in and out....But I dont really want to order a cable and use that as it is not really ideal....Was hoping to get sound working via the HDMI port?...Is there a way to disable the optical sound ports so maybe the HDMI passthrough for sound maybe kicks in?

and if your card is anything like that on (or even the 7600GT I used to have), there is no way that you were getting audio over HDMI as the DVI ports don't support it and there is no SPDIF connector on the card (to allow audio pass-through) like there is on the 8800 series and up,

I am going have to agree with stormy13, even if you have an HDMI out on the card there is no way to get audio through it with a pass-through connection.... So unless you are mistaken on the model of your card I am stumped.
do you have anything plugged into the front audio ports? if yes unplug it, could you also post a screen of your realtek control panel also are you comfortable opening your case?
Nice that doing a search Medion doesn't even turn up that model (8818). Anyway a little persistant searching and I found this,

rather nice of the guy in post #7 to copy and paste all of the specs for it. That said after looking it over twice I can find zero mention of that card being able to do audio over HDMI (and that card is very rare FWIW). The only thing it lists for audio is the following,
So unless the card has some kind of onboard audio of its own (like the ATI HD series) or a SPDIF header on it like the 8800 series (to connect to the SPDIF header on a sound card or on the motherboards onboard sound), I don't see how you managed to get audio over the HDMI connection.Home-cinema experience:

Medion MD 8818 - Cable Forum in the spec it does say 7650 with HDMI connection and 100% I really did have sound over the HDMI!
