I don't think its worth a system restore, their has got to be away to fix it.

Have you uninstalled IconPackager? Personally don't like it as it does weird things when trying to re-set, as I can see from your screenshot.

If not, uninstall, I think it gives an option to Restore Defaults, its been a couple of years and I'm not risking an install to find out!!

Delete your iconcache.db and clear your thumbcaches using Disk Cleanup.

Have a look here, it has worked before. Also scroll near the bottom for a fix that involves a registry key. The key at the bottom fixed it for the last poster I pointed to this post:

Instead of naming it .reg try naming it change.reg

Ok, let's see. I saw this on the link provided by A Guy. Not sure if it'll work, but it might be worth a try.

As it's still messing with the registry it is a good idea to make a back-up and restore point.

You could get into the registry editor and navigate to
    Code:
    HKLM\Software\Microsoft\Windows\Current Version\Explorer\Shell Icons
and then delete that entry. Log off and then log back on and see if that fixes it. Good luck!

Edit: What A Guy said above (at the same time I wrote this), but in more words, lol
