Have you uninstalled IconPackager? Personally don't like it as it does weird things when trying to re-set, as I can see from your screenshot.
If not, uninstall, I think it gives an option to Restore Defaults, its been a couple of years and I'm not risking an install to find out!!
Delete your iconcache.db and clear your thumbcaches using Disk Cleanup.
Have a look here, it has worked before. Also scroll near the bottom for a fix that involves a registry key. The key at the bottom fixed it for the last poster I pointed to this post:

Ok, let's see. I saw this on the link provided by A Guy. Not sure if it'll work, but it might be worth a try.
As it's still messing with the registry it is a good idea to make a back-up and restore point.
You could get into the registry editor and navigate toand then delete that entry. Log off and then log back on and see if that fixes it. Good luck!Code:HKLM\Software\Microsoft\Windows\Current Version\Explorer\Shell Icons
