Bug/Error? (Image not loading in My Documents)

Sorry about the title, I didn't really know what to name it. Anyways, here's a picture of my problem. I'm sure you'll be able to notice it right away. ;p

Is there any others like that or just that one?

Mine looks like this

Just that one. Also, it's odd because sometimes it loads like yours and other times (most times) it doesn't load at all (like the picture).

EDIT: Nevermind, I figured it out. Apparently, that image shows the program that opens the filetype of the file you have selected. So, if I click on a .txt file, it shows the notepad icon there.
