Thank you for your help Noel anyways. The problem is solved for the moment.
I had some other issues, so I had to restore from an image backups before the problem existed. I actually found the cause of it doing the backup & restore. It is very strange but if disk cleanup cleans the windows update files the problem happens, if I don't use disk clean up to clean up the windows update files the problem doesn't happen. It is showing no duplicates updates and I am not having any install issue right now.

This is a known problem with a number of updates - it seems they don't follow the proper logic and get removed by the DiskCleanup tool.
The current recommendation is to NOT use the tool at all until MS update either it or the affected updates.
