KB947821 is SURT [System Update Readiness Tool], it is a self-installing utility that executes when you install it. It is used because of an inconsistency found in the Windows servicing store that may prevent the successful installation of updates, service packs, and software. It checks for these inconsistencies [integrity] and tries to fix any issues found. It supports all versions of Windows 7 SP1.

[1] KB4457144 is the September Monthly Rollup, and like all previous Monthly Rollups it is cumulative. Have you tried installing the latest October Monthly Rollup KB4462923 from here => October 9, 2018 - KB4462923 (Monthly Rollup). Being cumulative, it will include any updates included in the September Monthly Rollup!
[2] KB4457144 requires the Servicing Stack Update KB3177467. You can however, download the KB3177467 .msu [Windows Standalone Update] manually from here => KB3177467 Servicing stack update for Windows 7 SP1. Then you can either download KB4457144 [September Monthly Rollup] from here => September 11, 2018 - KB4457144 (Monthly Rollup), or download KB4462923 [October Monthly Rollup] from here => October 9, 2018 - KB4462923 (Monthly Rollup).
IMPORTANT: Make sure you create a System Restore Point before doing any of the above!

So I manually installed KB3177467 first.
Then went to the October KB4462923. Microsoft had two files listed to download, one labelled 'pciclearstalecache....' so I ran that one first assuming that clears some stale cache.
Then i ran the October cumulative update but again had the same error. Computer restarts during boot. Have to go to safe mode which then rolls back the update.
Being a cumulative update Im guessing its a particular update or combination of updates causing the issue. I guess I could try installing each one manually, which might narrow things down, but even if I work out which updates cause the issue, Im still going to be in the same boat. Any last ideas before I reinstall?

It is used because of an inconsistency found in the Windows servicing store that may prevent the successful installation of updates, service packs, and software. It checks for these inconsistencies [integrity] and tries to fix any issues found. It supports all versions of Windows 7 SP1. When SURT is run, it will create two log files, CheckSUR.log and CheckSUR.persist.log, both in the temporary folder C:\Windows\Logs\CBS.
SURT downloaded => System Update Readiness Tool for Windows 7 for x64-based Systems [KB947821] - [October 2014].
[1] Install SURT [it will automatically run on installation and may take about 15 minutes or so to complete].
[2] Post the log: C:\Windows\Logs\CBS\ CBS.log.
[3] Post the log: C:\Windows\Logs\CBS\ CheckSUR.log.
[4] Post the log: C:\Windows\ WindowsUpdate.log.
SUGGESTION: It is advisable to turn off the power options for sleep etc while this is running because it could take a while.
I am sure that someone will be able to look at the logs and give you an answer.

Can confirm that July’s update also causes this issue. I feel like it has been happening since about then. So it’s likely something that started in this update.

Try this.
Uninstall KB4462923 and KB4457139 with the wusa tool [see below]. Then run WU. It should only find KB4462923 to be necessary. It has been suggested that doing it this way without KB4457139 being installed works!
[1] To uninstall a WU through Installed Updates => How to Uninstall a Windows Update in Windows 7.
[2] To uninstall a WU using the wusa tool from an elevated command prompt => Click Start, in the search box type cmd, when cmd appears in the menu under Programs, right-click it and select Run as administrator. The elevated command prompt window will now open.
Type => wusa /uninstall /kb:4462923 /quiet [yes, there are three spaces there] and press Enter.
Type => wusa /uninstall /kb:4457139 /quiet [yes, there are three spaces there] and press Enter.
If you use Copy and Paste, you will need to use the mouse right-click and paste option because CTRL+V doesn’t work in the elevated command prompt window.
