atikmpag.sys - is your Graphic driver, "ATI Video driver".

Also your BIOS is from 2009 i recommend updating this, as some of the new drivers/hardware can struggling to get along.

AMI | American Megatrends Inc. : BIOS and EFI Firmware

Please Run Driver Verifier, To narrow down the problem however I am almost certain that its your graphics driver.

Driver Verifier - Enable and Disable

There's no need to run Driver Verifier.

The ATI driver is the latest.

    Code:
    3: kd> lmvm atikmpag
    start end module name
    fffff880`04491000 fffff880`0451f000 atikmpag T (no symbols)
        Loaded symbol image file: atikmpag.sys

Download the 13.9 driver from here then use AMD Uninstall Utility to remove all traces of your current driver, CCC etc. Reboot and install the 13.9 driver.

       Note
When installing choose 'Advanced' or 'Custom' (whichever option is available, just not Express) and uncheck everything except the video driver.

    Report back with the results.
