All Desktop Icons refresh when starting SOME programs

Recently, started noticing this issue on an old Win 7 install. Icons all refresh when loading total Commander, Firefox, and other programs. Many programs do not cause the refresh.

A clean user profile does not do this, so it's something that got bolloxed in my setup.

Anyone know where to begin looking for the cause? I already did the IconCache.db delete thing, didn't help.

This is only happening on loading of programs, not randomly.

On Control Panel\All Control Panel Items\Personalization - Desktop item settings - un mark allow themes to change desktop items.
Not sure if it will help, anyway...
