atikmpag.sys - is your Graphic driver, "ATI Video driver".

Also your BIOS is from 2009 i recommend updating this, as some of the new drivers/hardware can struggling to get along.

There's no need to run Driver Verifier.

The ATI driver is the latest.

Download the 13.9 driver from here then use AMD Uninstall Utility to remove all traces of your current driver, CCC etc. Reboot and install the 13.9 driver.

   Note
When installing choose 'Advanced' or 'Custom' (whichever option is available, just not Express) and uncheck everything except the video driver.

Report back with the results.

when installing 13.9 driver, I have the custom to select only the HDMI audio driver

probably not compatible with the r7 260x
