I don't assume you have ever replaced the HDD?
Having a second look at the HDTune results shows that I've missed the benchmark. The benchmark result indicates a possible failing HDD, because the transfer rates are really low.

I would recommend to perform a system backup a.s.a.p., if you haven't made any backup yet.

Currently the best (only?) option would be to replace the HDD.
