BSOD - assuming the video cards the culprit

AMD Phenom II X2
Nvidia GeForce GTS 250

Hit by the BSOD, started out as a rare occurrence then became more frequent.
Usually occurs when video or just an audio file is playing.
Tried updating the video driver but that didn't help (tried it 3x). I should mention during the update process after the restart portion I received a message box "failed to load dll". After clicking the OK box the update would then continue and install. Even got the BSOD during one attempt of the driver reinstall.
So I'm still thinking i got an issue with the video card either driver or hardware, but I'm not ruling out other possibilities just yet.

Ran memtest and my RAM seems fine.
The towers well ventilated and nice and cool

Solutions, Suggestions, Opinions, Help appreciated.

Your .dmp file shows a stop error of 0x124 which is a general hardware error .

A "stop 0x124" is fundamentally different to many other types of bluescreens because it stems from a hardware complaint.

Stop 0x124 minidumps contain very little practical information, and it is therefore necessary to approach the problem as a case of hardware in an unknown state of distress.
