BSOD at startup and waking up from sleep, suspected Video Card Issue

BSOD at startup/wake up. switchable graphics Notebook.

Hello everyone,
I have a HP ENVY 15, 3040nr with switchable graphics (product specifics available at this link).

EXISTING SITUATION:
Switching between AMD and INTEL always happened with no problems, when you switch off the notebook it will switch back on with the last selected configuration.

PROBLEM:
Basically, few weeks (I suspect problem started after a Windows update, but after evolution of dynamics I'm not so sure) I started experiencing random BSOD at startup or when waking up the system from sleep (not hybernation), it expecially looked like that the BSOD was guaranteed when system was sent to sleep when Intel graphics were selected.
BSOD would happen, and system would restart with AMD graphics turned on by default.

FIRST TRY: WIN UPDATE. THEN FORMAT C:, REINSTALL ORIGINAL HP DRIVERS - NO RESULTS
I tried downloading updated drivers from HP for the Intel from Win Update (non existing) but problem kept showing up. I then formatted and reinstalled everything from scratch. This time I installed only the original drivers coming with the original hardware (you can find them on HP website), but the problem kept showing up.

SECOND TRY: CLEANUP, INSTALL UPDATED DRIVERS - NO RESULTS
So i formatted again and this time installed Catalyst_14.2_Beta_v1.3_UnifL, which are ad-hoc designed driver, making up for HP lack of updated drivers for this kind of device. Previous versions of this driver worked wonderfully for whole last year. They worked fine for a few days but I still got a BSOD

BSOD SITUATION:
Intel card was selected, I was cheking the screen properties on the AMD driver dialogs, and i got my Blue Screen.

FILE ATTACHED:
1)2) OLD MINIDUMP these are the minidumps from the BSOD I got before formatting. Maybe worth checking.

3)4) are DM LOG and SF Diagnostics zip files.

I will be really grateful if anyone could give it a look. Thanks in advance.

You appear to have issues with your HDD.
Please run a CHKDSK by following these instructions.

Then run Seatools to see if your HDD is failing.

CHDSK and Seagate General Quick Test both passed.
(HDD is Hitachi)
Did you check the two old Minidumps? If I remember correctly in those two the process involved in the BSOD was igdkmd64.sys . In the mean time I will try to replicate the original BSOD in order to post you a full report.

Enabling Driver Verifier can sometimes help. Please enable it by following these instructions.
When you choose the drivers make sure you don't enable ones from Microsoft Cooperation, Unknown or Macrovision

Second BSOD today

Thanks for your suggestions. I have not tried the verify driver step yet, I am gonna proceed now if you still recommend it.

But first please take a look at this:

1) I Switched on the AMD VIDEO CARD and afterwards sent the notebook to sleep.

2) WAKE UP FROM SLEEP and got this SERIOUS GRAPHICS GLITCH (see attached pictures)
basically the screen is "cut in half", the first picture shows the login screen after the wakeup. You can clearly see the artifact, which keeps happening just by moving the mouse above or below the horizontal line. Basic operations are still possible (in fact I logged in, as you can see in the second picture), but the artifact will persist.

So I switched off the machine, and upon shutting off the machine I got:

3) BSOD UPON EXITING WINDOWS (see attached picture and zip files)

I tried running the following command:
msinfo32 /nfo C:\report\msinfo32 /showcategories

but localization of the resulting file is still in Italian...

Did you enable driver verifier? It would really help.

Seems like I cannot change the Language with this windows licence, Multilingual is only available for Win 7 Ultimate...
I am trying to get the report done using EVEREST/AIDA.

Confirm: impossible to start-up windows, persistent BSOD even before windows loading screen. :banghead: I had two in a row before going back to safe mode and disabling verifier.

Here is the report. Hope you can manage to help me isolate the source and address the issue.

Can you try enabling Driver Verifier again as it couldn't find the faulty driver.

Of course. Just tell me (sorry if these are a noob questions...):
1) how many times should i let it try to boot? since It will be a sequence of BSODs and failed boot-ups...am I right to assume each one is going to leave a minidump?
2) can I use DM Log collector and/or SF in safe mode? This way i could provide you the minidumps more quickly.

Just let it try a few times then go into safe mode and use the DM Log collector. So yes it will work in safe mode. The only difference safe mode does is load the basic drivers and settings. All applications should work in safe mode.

Post scriptum: this is really strange. i deleted the content of c:/windows/minidump and I ran again through the startup sequence and got another four or five blue screens, but i found no more minidump files in the c:/windows/minidump folder. I made sure that Windows System advanced options had the "write minidump file" option ticked (I left it untouched since the beginning of today), but nothing got written. I'm afraid the files i provided you in last post are identical to the one you had already viewed.


(Moreover: I activated the "administrator account" but when i try to run DM collector from there in Safe mode, the .bat executable tells me that DM lacks administrator privileges and can't be run, so I had to go back to windows normal mode to get you the dump files)

Are you using your on board Intel graphics? If so can you try and disable it because it appears to be causing issues.
Disable it by going through device manager.

ALL crashes happened when the AMD graphic card was selected.

I just tried to disable the INTEL graphic card as per your advice, but the system INSTA-CRASHED on me as i clicked "disable". Attached you can see the result.

Right now the device is still active. I may try to disable it in safe mode.
