BSOD during random actions, no signal after computer sleeps

BSOD during random actions, APC_INDEX_MISMATCH most common error

Hello!

I am new to the forums but not to computers, been lurking these forums for a while for general info and good reads...

I have seen 4 BSODs since I did a fresh install of Winodws 7 Home Premium 64bit because I got a new HD through warranty exchange the other day. Also in the last 2 days I have noticed that if the computer is sleeping and I go to wake it, the monitor comes back as no signal. Computer running fine; fans, CPU, HD, etc cranking away just no signal. Only way to fix is on a reboot....I do believe these are related but I am not sure, it very well could be a power issue and I might just have to get another power supply... but that is the least of my problems at the moment.

System Specs:
Windows 7 Home Premium 64bit
AMD Phenom II X4 965 3.4 GHz
6GB RAM
ATI Radeon HD 5700 Series

-Last two BSOD have been directly related to this file, ntoskrnl.exe and the first two had to do with the following three files... atikmpag.sys, dkgkrnl.sys and dxgmms1.sys.
-Error message is almost always APC_INDEX_MISMATCH or KMODE_EXCEPTION_NOT_HANDLED
-Notice whenever I play League of Legends, almost always crashes once during that, other times it is random
-Since yesterday everytime computer sleeps, it does not "wake up" fully, no signal displays on monitor
-Error codes... 0x00000001 for APC_INDEX_MISMATCH, 0x0000001e for KMODE_EXCEPTION_NOT_HANDLED and 0x00000116 for the first two BSODs

Ran the Anti-rootkit Utility and no threats were found.

Going through process for the Windows Defender Offline, will report back shortly.

OK ... but there were some hints of virus infection, rootkit. As nothing is found, it is all good.

Now, install Driver Fusion. Uninstall all ATI/AMD display driver items using it, in safe mode. Then install one earlier version which was stable there.

Install earlier version in Safe Mode? Also can I use my driver disc that came with the graphics card? It is the earliest version and the one that was working (if I can remember correctly) before I had the issues.

Uninstalled all drivers, installed older version of drivers. NO BSOD so far, although they never show up this soon after a reboot. I will test the sleep issue now. What is next step?

EDIT: Sleep issue is no longer present. Just tested and computer now wakes up from sleep mode.

I'm sorry, totally thought I attached file, then deleted it from my desktop. Grabbing another dump file but I don't know how useful it will be since it's about 8 hours after the BSOD. Going to work now will be back later, my brother is going to create dump files if sees any BSOD.

That is not necessarily a guide but I guess I will start trying all the links and troubleshooting and see if that helps the issue. Quick question on BIOS... there are about 5 updates available for my particular motherboard but I am nervous about updating the BIOS, should I call BIOSTAR (my motherboard manufacturer) or what?

Okay did nearly everything in that guide... the only thing I haven't done is an overnight stress test which I am going to do tonight before I go to sleep, other than that everything is done.

One thing I did notice is that when I ran the SFC.EXE /SCANNOW that this came up...

"Windows Resource Protection found corrupt files but was unable to fix some of them."

When I try to view this log, it tells me access is denied.

UPDATE: Not as many BSOD last two days but computer did crash three times while trying to play League of Legends. Always seems to crash around same time into game, after first crash no issues even after you play a few games. It isn't even a blue screen of death but visual graphics and audio stuttering.

Did the repair install and noticed that it reinstalled that bad graphics driver that was causing issues earlier. Should I uninstall it and reinstall one that came with disc so I can revert back to old driver? Or should I leave it an see how it goes?

Repair install should not install any ATI graphics driver at all. That is on you after completing the installation.
