Bsod graphics_driver_tdr_fault 0116

At my wits end here. I cannot get this to quit happening. It only happens (for now) when I'm get into a video game. I have not installed anything new and this just started happening out of the blue today. I have done everything recommended in almost every post I've read here.

Voltages are fine. Memtests fine. No overheating problems. SFC scans all are fine. I have cleaned the drivers from safemode and done a full reinstall and also cleaned out old drivers. Updated the driver on the audio card. Removed EVGA Precision. No viruses detected from Avast or Panda scans. I'm running the 275.33 nvidia drivers. Anyone have a clue what I can do to fix this? If I need to rerun some tests please let me know and I'll post the results here. The minidmp info is below but I also attached the file if necessary as well as my specs. Thanks for the help in advance.

Okay I ran an OCCT gpu monitor test and everytime I do it I crash. I never see a blue screen it goes black and everything locks up and I have to reboot the system. I guess this means the damn gpu is bad? On a side note this is my second card in a month.

Still having the bsod 116 problem. I'll reseat the card and it might work a day maybe a few but then the screen will go black and I have to do a hard reboot. I have done new scans and attached them. I would appreciate it if someone could help me run this down. Thanks.
