The BSOD may be caused by an USB device, remove any unnecessary device

Code:

Probably caused by : USBPORT.SYS ( USBPORT!USBPORT_Core_iCompleteDoneTransfer+b2f )

Hi, Dono; and welcome to the forums. It may not be a USB device or your USB system. Something else might be interfering and conflicting. I find a some out of date drivers loaded on your system. Those in red font are quite obsolete. Outdated drivers can and do cause conflicts, memory corruption and BSOD. Update or eliminate the following drivers.
nvlddmkm.sys Thu May 14 16:48:07 2009 - nVidia Video drivers, Drivers - Download NVIDIA Drivers

nvm62x64.sys Fri Oct 17 17:01:06 2008 - nVidia Ethernet Networking Driver (nForce chipset driver), Drivers - Download NVIDIA Drivers

nvstor.sys Wed May 20 02:45:37 2009 - nVidia Storage Drivers, Drivers - Download NVIDIA Drivers

VSTBS26.SYS Wed Oct 15 20:54:06 2008 - Conexant HSF_HWB2 WDM driver, Conexant: Support

VSTCNXT6.SYS Wed Oct 15 20:52:22 2008
- Conexant SoftK56 Modem Driver, Conexant: Support

VSTDPV6.SYS Wed Oct 15 20:57:45 2008 - Conexant SoftK56 Modem Driver, Conexant: Support
How to find drivers. Please read this carefully and follow this guide.
- I have listed links to most of the drivers in the code box above. Please use the links there to see what info I've found about those drivers.
- search Google for the name of the driver
- compare the Google results with what's installed on your system to figure out which device/program it belongs to
- visit the web site of the manufacturer of the hardware/program to get the latest drivers (DON'T use Windows Update or the Update driver function of Device Manager).
- if there are difficulties in locating them, post back with questions and someone will try and help you locate the appropriate program.

- - The most common drivers are listed on this page: Driver Reference
- - Driver manufacturer links are on this page: Drivers and Downloads
- - To remove any left over driver remnants, especially from graphics cards, use driver sweeper Guru3D - Driver Sweeper
If you need help with this drivers, please post. Update these drivers, reboot and see how your system runs. Post back and let us know. If you get another BSOD after updating all of the above drivers, please upload it.

You don't need to format, at least not yet. Go to Device manager, find your old modem and uninstall it. Now remove the modem (if it is not an onboard modem). Using Windows Explorer, navigate to C:\Windows\System32\Drivers and look for those modem drivers. If they are still there after uninstalling the modem in device manager, rename each one from .sys to .old. Now reboot and they will not load. Once thos drivers are gone, run your system and see if you get another BSOD.
