BSOD NT Kernel & System

I have ran the memory test, and the Hardware Tests twice, and got no errors. I usually crash when i am gaming.
My computer is Windows 7 Ultimate 32-bit, I have re-installed the OS, but i still get the same results.
, This has been happening for a couple weeks i have read the other threads, and nothing has resolved i was hoping i can get some help , :D
I am using a ATI Radeon HD 3400 series video card
The mini dumbp file is attached using the windows_NT6_BSOD program,
