BSOD ntoskrnl.exe & ntkrnlmp.exe

Hello sevenforums community,

This is my first time posting. I came across BSOD problem that I have been having fairly often.
I tried replacing hard disk, reloading OS multiple times but the problem persists.
I ran WhoCrashed and below is the report I got.
