Solved BSOD on boot: APC_INDEX_MISMATCH. Also error 55 in system log

Hi,

My dell laptop started to experience BSOD when resuming from sleep. First only accidentally, later every time. During the last week the BSOD also happens during booting.
The error displayed is the APC_INDEX_MISMATCH code 1.
My system log shows this error during boot:
error 55 The file system structure on the disk is corrupt and unusable. Please run the chkdsk utility on the volume OS.

Here's what I have done so far, without succes:
- downloaded a microsoft hotfix for the APC_INDEX_MISMATCH but this hotfix didn't want to install, most likely because it is already installed in SP1.
- ran chkdsk
- ran sfc / scannow
- ran SeaTools 'fix all long' test
no succes so far.

Help would be greatly appreciated!

Install the NVIDIA DRIVERS 314.22WHQL only.

• Uninstall All nvidia items in Control Panel > Programs and features
  • 3D Vision Control Driver
  • 3D Vision Driver
  • Graphics Driver
  • HD Audio Driver
  • PhysX
  • nvidia Update
  (Are you using nvidia chipset drivers? If so, dont uninstall anything other than those are listed).
• Now follow Drivers - Clean Left over Files after Uninstalling
• Download 314.22 WHQL. While installing, Select Custom (Advanced) install. In the next page, follow this settings:

Better you run Seatools. If any disc error is actually found, there is a mechanism to attempt to recover it.

Free up the startup. Windows does not need any other program to auto start with it, but the auto start programs often conflicts and causes various problems including BSODs.

1. Click on the Start button
2. Type “msconfig (without quotes), click the resulting link. It will open the System Configuration window.
3. Select the “Startup” tab.
4. Deselect all items other than the antivirus and teh touchpad driver, SynTPEnh.
5. Apply > OK
6. Accept then restart.

Finally it is a suggestion, as I personally have never seen a single APC Index Mismatch issue that got really solved without uninstalling the third party antivirus. Though unlike the previous stop 0x01 issues, no clue is there in your case to believe that the antivirus is causing the issue, still as a caution, I would suggest you to uninstall Kaspersky using Kaspersky Removal Tool. Use Microsoft Security Essentials as your antivirus with windows inbuilt firewall, and free MBAM as the on demand scanner.

BSOD is back

I cheered to soon. The BSODs are back. It happens when I disconnect my laptop from my external monitor and kensington docking station in sleep mode. Resuming from sleep mode standalone results in a BSOD.

I have collected all required info using the SF diagnostic tool and uploaded it as rvaalten.zip.

Help would again be greatly appreciated.

I did everything you wrote

Still you have 25 items in startup! :sarc:

Reduce them as suggested at once, and then enable Driver Verifier to monitor the drivers.
Driver Verifier - Enable and Disable
Run Driver Verifier for 24 hours or the occurrence of the next crash, whichever is earlier.

Why Driver Verifier:
It puts a stress on the drivers, ans so it makes the unstable drivers crash. Hopefully the driver that crashes is recorded in the memory dump.

How Can we know that DV is enabled:
It will make the system bit of slow, laggy.

   Warning

Before enabling DV, make it sure that you have earlier System restore points made in your computer. You can check it easily by using CCleaner looking at Tools > System Restore.

If there is no points, make a System Restore Point manually before enabling DV.

   Tip

• If you fail to get on the Desktop because of DV, Boot into Advanced Boot Options > Safe mode. Disable DV there. Now boot normally again, and try following the instruction of enabling DV again.
• If you cannot boot in Safe mode too, do a System Restore to a point you made earlier.

Let us know the results, with the subsequent crash dumps, if any.

Its a Dell XPS L412Z (XPS 14 ultrabook), right? It seems that there is some issues with windows activation and that is causing the BSODs.
