Were the RAM cards in slots that passed memtest86+ before?
Could be motherboard or PSU and maybe a couple other things.
What PSU do you have?

My Asus Z77 recommends A2 and B2 (2nd and last) slots

Pcu is i5 3570k. The ram that failed was in the first slot, but i tested one of those after in the last spot and they passed. When i put just two in it was those exact slots that is recommended 2nd and 4th.

If you are running 2 sticks in those slots and still getting BSODs, submit the new dump file as specified in the http://www.sevenforums.com/crashes-debugging/96879-blue-screen-death-bsod-posting-instructions.html and run memtest with both those sticks in. I presume those are the 2 sticks that passed before.

Yes they were passing sticks. Here is new dump info

The sticks in memtest failed just like the 4 stick fail where it froze on test 8

Here's just a very brief section of the BSOD, we can see it was caused be memory_corruption, it doesn't necessary mean that a RAM module is faulty, it can also point to a object which corrupted the RAM at the time of the crash, for instance a corrupted driver or program. Paging files and the CPU caches can also be causes.

However, from the memory diagnostics we have carried out, then I think we can say there a problem with the motherboard or the RAM.

I realized that I had actually been using 4.1 but upgraded to 4.2 and ran the test with the two sticks again and got an error on second pass. Should I redo single slot tests with new new version of memtest?

In my opinion, you should continue running tests until you have convinced yourself, you have bad ram sticks or a bad board. Once you do that, you will know what course of action to take. Also, if it is the board and you decide to RMA it, Asus is going to have to be convinced.

Yes, I agree with essenbe. If you haven't you might confirm your BIOS settings are correct for your RAM, especially voltage and timings. Wrong settings can cause errors too.
