Clean up Program

When I got my first computer about 20 years ago a 486DX I bought a computer clean up program which was ok I guess. My computer is going on 3 years old so is there a program that will clean up and fix my computer. I know there are about a hundred of them but I don't know which one to trust or should I just reload the OS?
