Constant system freezing, every few seconds

Hey guys (and gals),
So the title really says it all. What I have tried to fix this (BTW I am running a triple monitor setup):

Restarting (obviously)
Running Chkdsk
Running Sfcscan now
Uninstalling and reinstalling CCC (AMD Catalyst Control Center)
Checking Task Manager for mysterious background tasks
Defragged

Thank you all for your help. I'm trying to get back into a decent game of CoD or LoL but currently it's unplayable.

The problem is present in safe mode. Sorry, forgot to put that in there too. I've been trying all day to fix this.

I did that clean boot as well, the issue still persists.

If you have the problem in safe mode, clean boot will not help.
We have to start making educated guesses. I think its your hard drive.
Go to the website of you hard drive manufacturer and test the hard drive.

So I went back yesterday to run the longer test on my HDD, and mysteriously this issue has ceased to exist. Weird. It left just as quickly as it appeared... needless to say I am very glad it left.

If this happens next time, ensure that you are running your HDD in AHCI mode, if you do, try updating those drivers, I had an issue like that and driver upgrade was my solution...

Cooling solutions:
- AVC @ 2000/5000 RPM Copper Heatpipes (For Athlon 64 X2 6000+ CPU used in an Athlon 64 X2 5200+)
- Rear Fan 80 mm @ 2700 RPM for heat extraction
- Manhatan Chipset Cooler @ 4700/7200 RPM (For nVidia Chipset in MoBo)
- Foxconn @ 2500 RPM (Old Pentium III heatsink fan) in XFX ATI Radeon HD 4350
Let me explain what probably happened. Part of the check is to replace corrupted segments. This can be done only a limited number of times. If it is needed to happen often, you will need a new drive. I suggest, back up your stuff now.
