Disk on Module issue [help needed]

Ok, I have been trying to figure this out and sadly searching hasn't helped (thats also how i ended up here lol), but here is my situation.

I have a Disk On Module (will refer to it as DOM here on in) and I was able to use the darn thing for awhile, then it stopped working for a bit then some how (don't know what happened) it suddenly decided to work as it was supposed to. so I reformatted it so I could erase the data off it and use it as I intended, to store pictures, however it stopped working again (thank goodness for my habit of backing up) and what happens is this, the windows 7 x64 ultimate see that it is there (drive "F") however I CAN NOT get it to reformat, and disk manager does not let me do any thing with/to the drive, so I need help figuring this out...

its really frustrating to see that the computer can see the drive and knows its there but I can't even get chdsk to run (gives "chdsk not avalible on RAW drives" message at boot) at boot...so yea I throw my hands up and say "HELP"! lol I have racked my brain going to the ends of the inter tubes trying to solve this, so far im 2 for 2 on DOMs and them not wanting to work...

There are several 3rd party apps (free trial) like paragon disk suite that will do it.

yea I have been searching for the best one, any suggestions? maybe links? other ideas if that fails me?

ok heres an update, as i dont care about what was on the drive (have that stored on another drive backed up anyways) i tried to reformat the drive, no go, so i started trying to split the drive into multiple partitions which worked, got a 1 gb partition in NTFS the other was RAW (this is a 16GB drive) so i tried again same thing, but format failed and ALL space went to "unallocated" and the drive is no longer wanting to initialize at all, so now im at a loss as to what to do, this has happened before to me once, but the issue some how resolved its self before...im getting to the point where im just ready to remove the drive, reboot, then reinstall the drive and hope it works....uuuug....

bump........any one? I just attempted to use linux to reformat the SSD device and still no go...thinking it may have gone bad?
