Drive Corruption 0xc000000f on Win Boot Manager 2nd time

One of my machines, for some reason, keeps corrupting the boot drive.

Custom built Intel i3-3245, 8 gigs memory, 250Gb HDD, EVGA 950Ti, Win7 64 bit. This machine has been up since 2013. I added an Samsung EVO 850 to it Sept 2016 and it's been working ok with an occasional BSOD (can't recall the code, but it was pointing at a hardware problem), but otherwise been great. Last weekend, it BSOD'ed and upon reboot got the Windows Boot Manager with a 0xc000000f an error occurred while trying to read the boot manager. I switched back to the old 250GB hdd in the system and it booted back up ok. So I got the Win7 disc out, and went through with bootrec /fixboot and /fixmbr, did chkdsk on boot, it tried to fix permissions issues galore across 500k+ files, etc. I got it all the way to at least find the disc again at the right size, and the files were back, but the registry was also corrupted, so I finally gave up, and did a re-install and got everything back up and working again. Sunday, the drive corrupted yet again. Same boot message again. System again sees it as a 0MB drive. System wants to chkdsk again. So something keeps corrupting a LOT of the drive. But again, I can boot back to my old 250GB drive.

I looked at it with Samsung Magician. SMART data shows good. It finds the drive perfectly fine, and I can run benchmarks against it.

So only thing I can think of this is is the drive is bad, the SATA cable is bad or the SATA controller on the motherboard is bad. I also suppose a voltage spike to the drive could be corrupting it as well. The PSU is a CoolMax CR550B which is likely around 10 years old. Voltages in UEFI show slightly above voltage on 12, 5 and 3.3v lines so that looked fine at least. But it is old. It could also explain the occasional BSOD's as well.

Right click to run as administrator (XP users click run after receipt of Windows Security Warning - Open File). When the tool opens click Yes to disclaimer.

Make sure that Addition option is checked.
Press Scan button.
It will produce a log called FRST.txt in the same directory the tool is run from.
Please copy and paste log back .
The first time the tool is run it generates another log (Addition.txt - also located in the same directory as FRST.exe/FRST64.exe).

looks like Samuria was right in asking you to run FRST.

First off your running 2 AV's get rid of 1, your choice.
i can see that 1 is sytsem the other firewall they still dont play together well

Within the report however there are other issues

You have an 2nd Administrater account named Test is it yours,(not Dante)
Wajam is classified as malware
you have numerous java entries with no files
pcalua is a valid MS prog - but on your system its in the wrong place
at some time you have run a tuneup utility PCkeeper there are remnants.

We can remove anything marked >>>Attention or no file.
But im more concerned about Test.

Please keep in mind, the focus of my problem is why my drive keeps corrupting all of a sudden. All of these things will be wiped and start from scratch (again) when I either figure out what's going on with the current drive or buy a new SSD to replace this one if it's failing. :) I couldn't run this on my SSD because even though I got chkdsk to recover the drive and it can start to boot, it bluescreens so fast I cannot even see the error and have to fall back to my old drive to boot to. I'm just trying to figure out what I need to replace/fix if it indeed is a hardware issue. :( I'm just not seeing solid evidence of a hardware issue yet though.

I would say that you have a defective drive.
As you have the old HDD for windows, try to install a Linux on the SSD. If the Drive Corruption happens again (this time under Linux) you know that is a hardware problem.
Samsung SSD 850 EVO has 5 years warranty.
To install Linux, don't forget to detach the HDD from the MB.

But how can I get Samsung to come through on the warrany if the drive isnt showing any sign of a problem though that would allow me to make a claim?

Argh! I replaced the Samsung with a Western Digital Blue SSD about 3 weeks ago and everything has been smooth.... until last night. Got a Stop 0x124 BSOD, and it had to do chkdsk again, with major major corruption. For whatever reason, the index and thousands and thousands of files get orphaned. Any ideas what to hit next?

Clean install, updated everything from scratch. No recovery from a backup either.

It can be a bad contact. Try to change the SATA cable, the SATA port and the PS connector (switch between the ODD and the SSD).

I ordered some SATA cables today since I'm out. I'll try a different SATA port next time around with a new cable. I have multiple full boxed copies of Win7. I suppose it could be the disc, but it did run fine for 3 weeks before this happened. I could try a different one next time.
