I use ESET smart security only, is it enough ? or I have to get another antivirus? spyware ?

It has been enough for me for 3 years although I do a scan with malwarebytes every now and then. It always comes up clean.

ESET smart security is enough. If required, do a weekly scan using MalwareBytes or Superantisypware.

ESET + MSE (real time protection).

No, this is a terrible idea. Never have more than one real time protection A/V suite running at once. Your system is bound to become unstable running both ESET and MSE at the same time.
