You are using an older, weaker GPU and playing with 1080P resolution.

I suggest lowering down the resolution to let's say 1024x768 and put everything on low. If that doesn't help then it's time to get a new graphics card.

Or possibly integrated graphics if the op Doesn't have the budget for GPPU! :D
