My screen freezes then goes black every 5 minutes

It if very annoying because I am trying to do shit and then my screen will freeze for a few seconds, then go black for a few second, then flashes a couple times, and then starts to work again. the entire process takes about 15 to 20 seconds. The error message that comes up says

Display driver stopped responding and has recovered
Display driver NVIDIA Windows Kernel Mode Driver, Version 295.73 stopped responding and has successfully recovered.

I've tried re-installing the drivers for my card but it didn't work. I have an NVIDIA Quadro NVS 420.

Download this NVIDIA DRIVERS 296.35 WHQLthe latest driver for your GPU. Then Download this Phyxion.net - Driver Sweeper and remove your current driver. Install the new driver.

If you have multi DIMMs of RAM, then try running your PC with only one at a time. Completly remove the drivers and reinstall new one.
